I don't think that's how Medi-Cal works - I am not absolutely certain, but this seems clear - even with an exemption to be on straight Medi-Cal, I don't think you can force Keshishian (or Rabkin) to be your surgeon. If they are not contracted with Medi-Cal, you can't get to them using Medi-Cal. There is to my knowledge nothing to fight.
I guess Keshishian is only contracted with Medicare, so you might have to get on Medicare to get access to him. Or, you may have to deal with Coates and INSIST that you are not given a SADI, and put it in writing at each stage of the process.
