The more malabsorbation and lot more complications
Complications happen with both. Best to have a good surgeon vetted in both surgeries. IE: not one who’s only done a handful of DS surgeries.

Malabsorption is the idea behind the surgery. the RNY loses the ability to malabsorb food, but still malabsorbs vitamins.

It all depends on how you look at it. Weight regain is a much greater problem with gastric bypass than with DS. I consider that to be a complication. Look carefully at the post-op diet your surgeon recommends, not just the first couple weeks but for the rest of your post-op life. Take an honest look at yourself, and ask yourself it that's something you can realistically stick to permanently. For me, it wasn't, and I knew it.
Post-op eating with the DS is much easier than with gastric bypass, due to the greater and more permanent malabsorption. I consider this a plus, not a minus. Yes, there are greater needs for vitamins, minerals and protein, but these are very doable for most people. You will need to follow your labs carefully with either operation, and adjust your supplements as needed. There is no getting around that with either operation. I've lived with the DS for about 18 years now, and have no regrets.
Learn everything you can about both operations, especially the long term results. Then, make a well informed decision as to what is best for you.
