Hello everyone, I'm sara and just started on this website. I was approved for a revision to the rny. But after that decision I researched more and found the ds was what I really needed. So back to the doctor I went and discussed the ds with him. He seemed okay to do this. Well I received a letter from him today stating that he had taken this to the team meeting and felt that I should wait to prove I was going to do what I needed to. first he wants me to go to my primary and have an endocrinology test. this because I shared I gained 50 pounds in 1 month. I also am to go to the dietician and physiologists for 3 months and then the 3rd month I also will see the psychiatric doctor. And then they will take it back to team. Needless to say I was hugely disappointed. I do not understand. It was okay to go ahead with the rny but they feel all the sudden that I am not seriously commited to doing what I need to with ds. please everyone please share with me I am so in the dumps about this. I think its unfair. but what do I know, I did my research and I believe I`ve picked the best option for me. This is so important to me I could just cry. thanks for your ear. Sara
 
So what are you revising from?

The requirements for RNY, DS and sleeve are all the same, so I'd have an issue with this. Who is your surgeon, and is it possible to switch?
 
Hello I am revising from a sleeve which was great, but this year I have gained. My surgeon is Dr David Shumacher Kettering ohio. I suppose I could look elsewhere do you have any suggestions for ohio.
 
You didn't get kicked in the stomach. You got lucky. Now you can get the RIGHT surgery for you, with a qualified surgeon. You actually dodged a bullet.
 
You lost me, how did I dodge the bullet. I was going to get the ds now I have nothing. evidently there is no one else in ohio that does this if I go futher out how would I make follow ups etc
 
You get your surgery wherever you can get the best surgeon; you follow up with your PCP. It is best if you can find a bariatric surgeon who at least knows SOMETHING about the DS if a problem arises, but not the only way to manage. I'm not sure whether either of Teel or Maguire are still practicing there, but Margaret Inman is available in Carmel IN.

You still haven't said what your current surgery is - if it is a crapband or sleeve, that's completely different (less difficult) from an RNY to DS conversion - if just a conversion from band or sleeve, I think I would go to Inman for surgery.
 
If my insurance covered bariatrics, Sara, I would consider Inman (I'm in Ohio).
 
I just cringe when a surgeon is encouraging a sleeved person to get RNY. I see it all the time - what's easiest for them seems to trump a lifetime of what's best for the patient.

It's not a big deal to get follow up if you travel to your surgeon, in fact I went to India from Washington state. My pcp does my follow up, and at 7+years out is usually just annual labs. I forward those to him, but my pcp addresses any issues on them, or refers me on to a specialist (not like I'd want my bariatric surgeon addressing my thyroid or kidney stones or whatever).

I'd look for a legit DS surgeon, since going from a sleeve to DS shouldn't be a big deal - nothing like reversing RNY to DS.

Sara, I just got your pm, so I don't know what happened to the previous ones you tried to send. I was not ignoring you, I promise!

A couple things...somehow the surgeon's name is familiar to me, so it's possible he's done the DS before, but he definitely isn't one of the names we see all the time. Whether he's insisting on more hoops to jump through because he's not comfortable with the DS or because he is worried about your ability to follow the rules, I have no idea. But it does seem odd to me that he would be just fine with revising to RNY and not so fine with revising to DS. Both operations require specific vitamin, mineral and protein requirements for the rest of your life - more for the DS, but the concept is the same.
One thing I do agree with him on, through, is some kind of endocrine assessment. It is not normal to gain 50 lbs in 1 month. There is no way you could have eaten enough calories to gain that much weight so fast. So, either you are retaining huge amounts of water all of a sudden, or you are taking some medication that causes weight gain (corticosteroids, for example) or there is some kind of endocrine problem. This is NOT a reason not to have the DS, but it's something that should be sorted out before hand.
Or, I guess someone could have misread the number of the scale. Do YOU think you've gained 50 lbs in a month?
I think, at this point, you need to open your mind to going elsewhere for your revision to DS. Your closest option is probably Dr. Inman, who has an excellent reputation and definitely has lots of DS experience. I can't possibly know all the facts or what is going through this surgeon's mind, but I will respond to your sense that this is unfair simply by saying that life is often unfair. That doesn't make it better or right, but it's a reality of life. So dust yourself off, pull up your big girl panties, and figure out how to get yourself to Dr. Inman. The extra effort will be well worth it.

I hear you I went to her website I'm unsure about my insurance and going out of state. plus since my insurance has approved the rny how will they take a new surgeon out of state
 
the insurance person in her office may be able to answer your questions about insurance. If not, you will need to get a copy of your EOC (evidence of coverage - a lengthy document, not a summary) and sort it out yourself. @DianaCox and I can help with that if needed - and she's better at it than I am. But that's where you start.
