Letter for new PCP.....where can I find it????

A while ago someone posted a letter they had written for new PCPs that didn't understand the DS. I have looked high and low for a copy of that letter (and yes I have used the search function) but I can't find a copy of it. Can someone help me please! I have my first PCP visit post-op today and I can't see my regular doctor so I'm getting stuck with the PA.
