Help with appeal letter

Dr Keshshian and Kristina informed me that the insurance denied me because "severe dumping syndrome" is not sufficient enough reason for my insurance to approve the revision. Hey ask dr. K his professional opinion but because he has never met me face to face he isn't allowed to give his professional opinion therefore isn't allowed to do a peer to peer with insurance. So Christina told me my best chance was to write an appeal to make sure that the letter clearly states complications complications complications nothing about I need the surgery to lose weight. HELP!! PLEASE!
