Does anyone have Dr.Keshishian's Lab request order?

My yearly PCP visit is tomorrow. I have a copy of Diana's list from Rabkin and a copy of @southernlady list.
What I am not seeing is the urine test I know was on Dr. K's annual lab's and I lost it.

@Barb1 you may have to contact his office. I don't typically have a urine test. That depends on what my PCP wants to do. When I have had one, it's been part of my yearly physical to check kidney function since I am still classed a diabetic.

Thanks Liz. I can't believe I lost it. I think I found it here at AACE Guidelines. I also looked back at previous bloodwork from 1 year ago and found a test for urinalysis ,complete w/reflex TC. I just want to make sure she orders everything the first time so I don't have to fast again on another day.
