Generic DS Lab order

If you don't have one from your surgeon (hell, even if you do), take this one to a GOOD PCP/Endo/Hemo, whatever...someone willing to pull all needed labs.

This was compiled based on two different surgeon's lab orders, the Vitalady suggested list and run by MajorMom.

This needs a MMA and homocistine also to verify B12 level. Also prealbumin, and Vitamin E

Rabkin's didn't have MMA and homocistine, E, prealbumin. But I still am not able to open the bariatric generic one. Could it be because i am on my cell? But other stuff opens using Kindle or Polaris. This just flat out says "cannot open file"
 
The link I clicked on for the drop box is just page one of one and just has the dx codes.
 
It is just one page...I took that to my PCP and he generated one based on that one. It's basically just a list of what tests and the codes for it. Many PCPs have their own or the lab they prefer does which is why it is such a straight forward listing.

I appreciate the lab order samples. But now do you have a cheat sheet of sorts to tell me what my levels should be in each....D, E, PTH, Calcium or bone ?

Also, looking to find brands that have worked well? What your daily vitamin regimen is (I realize everyone is different but just trying to get a feel for what I'm looking at)?

Just revisited and still cannot open this in dropbox. Says "cannot open file."
 
southernlady...if you have a copy of some results based off those lab orders can you look to see if the CRP (c-reactive protein) says HS (high sensitivity) or if it says quantitative?

There are two different types of CRP's and i'm trying to figure out which one it is.
