Need help insurance denied my revision

My insurance denied my revision for not a medical necessity. what can i do to get them to cover this.

Medicaid, which state?

You need to get the Medicaid policy that applies to you. And we need to see what the request for preauthorization from your doctor said, and exactly what your denial says to explain why you were denied.
