Lab Result Chart

I have tried the search function to look for a chart or graph that can be used in Microsoft Office Excel 2007.
Somewhere I read there is one to keep track of our lab results. Does anyone have are know where I can find this?

We used LabCorp as our ranges but if your lab isn't LabCorp, use the ranges that your lab does use.
