is the provider website medical mutual-- policy statement on bariatric surgery for obesity,,, the same as evidence of coverage that you recommend to get??? i could upload the policy if that helps... trying to decide if i have to request the eoc or not?

IF you are signed into your portal for the medical provider, it might be. If you just go to their web site and are not signed in, no it is not.

And yes, you need the EOC for YOU. If it's thru a company, request the EOC from your Human Resources department.

thank you. I have gotten insurance approval and surgery scheduled for sept 18, that was the soonest date available. The dr office was booked up for 3.5 mo after i had insurance approval
