Insurance denial twice need help

I am New still trying to figure out how to post in here, I was told you could help me, I was denied DS surgery, I was wondering could you help me with a appeal letter. I was denied for not medical nessecity, AND non compliance with my RNY surgery I went to first appointment post op, I lost my medical insurance and was unable to go to baractric Dr just couldn't afford it but I went to my PCP doctor my RNY was in 2003, I gained all weight back,I'm PRE diabetic,GERD,knees hurting back etc. Can you please help me write appeal to get me approved

Good morning - I'm so sorry you are going through this. I had my DS Revision surgery in January, but I was denied by insurance as well. Please see posts on this. I shared my appeal letter which may give you some ideas. I had to appeal it all the way through external appeals, but was finally approved. Best wishes.
